I read the Nob Hill stuff and I think that your recourse is the tip. If by protesting by the tip you encurage good wait staff to leave then you are doing the world a service and contributing to the greater good. So, go to an ATM, get cash (make change nearby) and pay the tab with no or little tip and walk out. It's harsh but they are clearly exploiting your common courtesy and sense of established manners.
